Former England international Terry becomes the first ever Chelsea player to achieve the feat since the Premier League began in 1992.
His impressive feat means the 34-year-old joins Gary Pallister, who won the title with Manchester United in 1992/93, as the only other non-goalkeeper not to have missed a second in a Premier League championship-winning season.
