Chelsea are planning to get rid of Diego Costa by facilitating a player-swap deal involving Atletico Madrid.

Spanish publication Fichajes suggests the Blues are keen to rid themselves of the underperforming Costa by sending him back to former club Atletico and bringing Jackson Martinez to Stamford Bridge.

Brazil-born Spain international Costa has scored just three goals in 14 Premier League goals for Chelsea this season and has recently been linked with a winter departure form the reigning English champions.

It is believed that Diego Simeone is keen on a Costa return and is prepared to part with Colombian international Martinez who has netted just two La Liga goals since arriving from FC Porto for €35million in the summer.
