Bologna are believed to be prepared to offer Emanuele Giaccherini a return to Italy.
The 30-year-old winger has endured a difficult spell in the north east since arriving on Wearside from Juventus in the summer of 2013.
Giaccherini's agent Furio Valcareggi has revealed that his client is pondering a move to Bologna having started just twice for the Black Cats in the Premier League since last summer.
"We had a rethink on the situation that has been created in England," said Valcareggi.
"He is certainly not lacking for offers. He could decide his future within two days."
21-time Italy international Giaccherini is desperate to play regularly so he can force his way into Antonio Conte's Azzurri squad for next summer's Euro2016 in France.
Valcareggi added: "He lost all of last year and now cannot afford to lose another chance. This is the year leading up to Euro 2016 and he really wants to be there."
